Responsibilities
Skills Development & Coaching
- Provide structured coaching and guided practice to support participants in developing independent living and community navigation skills.
- Model and reinforce strategies that promote organization, problem-solving, and effective communication.
- Support participants in building confidence and self-advocacy skills across various settings.
- Facilitate skill acquisition related to accessing services, managing responsibilities, and engaging in community systems.
- Adjust level of support based on participant progress and demonstrated independence.
Community-Based & Virtual Support
- Deliver services in home, community, or virtual environments as appropriate to participant needs and authorized service design.
- Provide real-time coaching and reinforcement during community-based activities when applicable.
- Encourage participant-led engagement and decision-making whenever possible.
- Promote increasing independence through structured support and skill reinforcement.
Family Collaboration
- Collaborate with family members and natural supports to reinforce skill-building strategies.
- Provide guidance that strengthens participant independence within the home environment.
- Support development of sustainable natural support systems.
Documentation & Communication
- Maintain clear and timely documentation reflecting services provided and participant engagement.
- Record observations related to skill development and progress toward greater independence.
- Communicate relevant updates, barriers, or concerns to the CFS Coordinator or Supervisor.
- Participate in team meetings as required.
Requirements
Qualifications
- An associate's degree in a human services field of study OR A minimum of three (3) years of experience in the developmental disabilities service delivery system
- Ability to work independently in home and community environments.
- Valid Driver’s License (if travel required)
- Must pass a DOJ/FBI background check and provide valid CPR/First Aid certification (prior to direct service)
Preferred Skills
- Experience working in social services, healthcare, nonprofit, or community-based organizations
- Experience working with individuals with developmental disabilities.
- Familiarity with Google Workspace, Canva, or basic CRM systems (Zoho a plus)
- Ability to work independently and follow instructions accurately
- Clear written and verbal communication skills

Pay: $24.19 - $29.13 per hour
