What are the responsibilities and job description for the Behavioral Health Liaison - Family Court position at Community Behavioral Health.?
Essential Functions:
- Coordinate all aspects of referrals as assigned including tracking, documenting referral status, notifying appropriate parties once provider is identified and following up with members/caregivers and/or providers to ensure services have been initiated.
- Coordinate and share information between respective CBH clinical teams and system partners.
- Meet with members to provide encouragement, emotional support, and positive feedback to reinforce healthy behaviors.
- Conduct community outreach, psychoeducation and engagement with CBH members.
- Assist members in identifying and addressing barriers to treatment.
- Conduct follow up with CBH members in the community and by phone.
- Participate in interagency meetings.
- Maintain an accurate daily log of work completed
- Maintain documentation in CBH’s electronic system consistent with agency and industry standards
- Come prepared for and actively participate in supervision.
- Attend assigned meetings.
- Cover work when peers are out of the office as assigned and find coverage when absent.
- Contribute to special projects and other duties as assigned.
Team Specific Responsibilities:
- Interview Court parties, eliciting demographics and presenting problems.
- Formulate plan for outpatient intervention and refer CBH members to behavioral health providers.
- Refer the uninsured to respective Base Service Units.
- Facilitate case handover to BHFEC (Behavioral Health Forensic Evaluation Center/PMHCC) staff for scheduling of evaluations mandated through Court Order.
- Facilitate implementation of behavioral health outpatient recommendations from Evaluations.
- Provide intensive case management coordination to clients and families and their auxiliaries.
- Monitor client’s compliance with treatment.
- Troubleshoot cases in which a prescribed component of care is absent or compromised, alerting point persons to the problem and maintaining contact with all parties until resolution.
- Interpret case histories and define services, in answer to inquiries of attorneys, child welfare partners, and others within 1 business day.
- Document all work within 1 business day.
- Apprise all Parties of arrangements for behavioral health services and of compliance with those services.
- Complete Court Reports.
Position Requirements:
- Education: Bachelor’s degree in Social Work or related field
- License/Certification: PA Child Abuse Clearance required
- Relevant Work Experience: Minimum of 2 years of experience in direct service including assessment, counseling and coordinating services
Skills:
- Familiar with services for adults and children in the managed care system
- Excellent interpersonal and collaboration skills
- Excellent verbal and written communication skills
- Proficiency with Microsoft Office and able to type 40 words per minute
- Strong organizational skills with the ability to multi-task in a high-volume environment
- Ability to work independently and as part of a team
- Compliant with HIPAA regulations
Work Conditions (including travel, overtime required, physical requirements and occupational exposure):
- Must be able to travel within Philadelphia and surrounding counties.
- Regularly requires walking, standing, stooping, bending, sitting, reaching, pushing, pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
- Ability to utilize computers and office equipment to complete daily work responsibilities.
- Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
- Must be able to adapt to continuous changes/demands of the job.
Philadelphia Residency Requirement:
- The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
