What are the responsibilities and job description for the PATIENT CARE COORDINATOR position at COMMUNITY AND RURAL HEALTH SERVICES?
Community Health Services is seeking a dedicated Care Coordinator to join our team! This is a full-time position Monday through Friday with no weekends and 10 paid holidays. The Care Coordinator will focus on targeted patient populations to achieve measurable improvements. This position works closely with clinical teams, Quality staff, the EHR Administrator, and community partners to drive evidence-based care, enhance care coordination, and expand remote patient monitoring programs.
Hours for this position are: Monday 7a-7p, Tuesday- Thursday 8a-5p, & Friday 8a-1p
What We’re Looking For:
✅ Registered Nurse (RN) with active Ohio License
✅ Minimum 5 years of experience in a healthcare setting, FQHC experience preferred
✅ Strong understanding of chronic disease management (diabetes, hypertension)
✅ Experience with EHR systems and population health tools (e.g., Azara)
✅ Data analysis, reporting, and performance tracking skills
✅ Ability to manage multiple priorities and meet deadlines
