About Us
Community Agency for Senior Citizens (CASC) assists Staten Islanders age 60 and older to live as independently as possible in the community. We do this by working hand in hand with older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage and recognizing the importance of friends, relatives, and community in creating supportive environments for people as they age.
Purpose and Objectives:
Supervise case managers working directly with older adults ages 60 and older in order to identify client’s needs and service preferences and work with client (and family, when appropriate) to develop implement and oversee a care plan to meet mutually agreed upon goals.
Responsibilities
· Supervise up to 5 Master’s and Bachelor’s level case managers
· With Program Director assigns cases to case managers.
· Authorizes EISEP homecare, housekeeping services and home delivered meals as per need and eligibility requirements
· Carry caseload as assigned by Program Director
· Maintain up-to-date knowledge of and working relationships with community resources
· Provide in service training related to case management skills and functions, monitor staffs’ data input into the DFTA STARS database and written case notes
· Maintain statistical and programmatic data as required
· Maintain familiarity with all DFTA standards including updates to standards and procedures
· Represent CASC at community events committees and coalitions
· Assist with other program activities as assigned
· Conduct home visit assessments for high-risk clients as needed
· Conduct supervisory reviews
· Evaluate employee performance and identify training resources and opportunities
· As directed by Program Director assist with supervision outside of assigned team when necessary due to vacancies.

· Program record keeping is detailed accurate and up to date
· Continuous quality improvement goals achieved
· Case management clients receive high quality, individualized services and support to help them remain safely in their homes and communities
· Effective implementation of cask procedures and guidelines
Requisite Skills and Capabilities:
· Excellent written and verbal communication skills detail orientated
· Leadership: ability to identify strengths and weaknesses in staff assigned and provide supervision, education and guidance to improve their job performance

Supervision and Education:
· Supervised by the program director
· Participate in team meetings
· Participate in DFTA and CASC training and education sessions
· Participate in local educational and informational sessions and conferences
· Annual personal review
Position Qualifications:
· Master’s degree in social work or related Master’s level degree
· Minimum two years of experience in a social service organization working directly with older adults
· Social work supervisory experience preferred
· Fluent in a second language preferred
· Crisis management skills and excellent communication skills
· Experience with DFTA contracts preferred
Job Type: Full Time in office Monday through Friday
Salary: $70,000-$74,000 annually
