What are the responsibilities and job description for the Case Manager position at Community Action Region VI?
Community Action Region VI is currently seeking a Case Manager to assess, plan, coordinate and monitor services for individuals and families facing various challenges within our service area. Advocate for clients and connect them with resources and support services to address their needs effectively. Works closely with clients to develop individualized action plans, provide ongoing support, and empower them to achieve their goals towards self-sufficiency and improved quality of life.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Work collaboratively with all staff to maintain compliance with all program regulations.
- Conduct comprehensive assessments of clients’ needs, strengths, and barriers to self-sufficiency.
- Develop personalized action plans in collaboration with clients, setting achievable goals and timelines.
- Coordinate and facilitate access to community resources, including but not limited to housing assistance, healthcare services, employment training, education, childcare, and transportation.
- Provide ongoing case management support to clients, including regular check-ins, progress reviews, and adjustments to action plans as needed.
- Maintain accurate and up to date client records, case notes, and documentation in accordance with agency policies, procedures, and regulatory requirements.
- Monitor and evaluate client progress towards goals and collect data to measure program outcomes and impact.
- Adhere to the Community Action Code of Ethics and confidentiality policy.
- All other duties as assigned.
QUALIFICATIONS:
- Bachelor’s degree in social work or related human service field preferred.
- Prior experience in case management, social services, or related field preferred.
- Knowledge of community resources and social service programs.
- Excellent organizational skills and time management skills.
- Strong communication, interpersonal, and advocacy skills.
- Ability to work effectively with individuals from diverse backgrounds and cultures.
- Hardworking, dependable, and able to work independently or as part of a team.
- Professionalism and discretion when handling confidential information.
- Valid driver’s license.
This position is 40 hours per week, Monday – Friday 8-hour days, with full benefits:
