What are the responsibilities and job description for the Medically Vulnerable Care Aid position at Community Action Partnership of San Luis Obispo County?
The Medically Vulnerable Care Aid will work with homeless individuals at the 40 Prado Homeless Services Center who require 24-hour supervision due to recent discharge from a hospital or nursing facility, or those who have graduated from the Recuperative Care Program (RCP). This role involves providing support to participants in medically vulnerable programs, including short-term post-hospitalization and Enhanced Care Management. Responsibilities include direct services such as assessment, information and referral, distribution of basic supplies, and on-site client supervision. The aid will implement treatment plans, assist with activities of daily living, and collaborate with shelter services staff to stabilize clients' physical health needs. The role requires coordination with multiple service providers and engagement with emergency services personnel to ensure comprehensive care. The Medically Vulnerable Care Aid supports CAPSLO's mission by encouraging and facilitating client self-sufficiency.

- Requires an AA in human services or medical programs, or a minimum of 2 years of experience providing direct human or health services to the economically disadvantaged, physically/mentally disabled, substance abuse/dual-diagnosed individuals, the elderly, and/or at-risk youth.
- BA degree in human services preferred.
- Nursing Assistance and/or Medical Technician Certification preferred but not required.
- Education, volunteer experience, and/or other related experience may be accepted.
- Depending on the department assigned, a valid California driver's license, a current DMV driving record, and proof of insurance may be required to fulfill job-related travel. Reasonable accommodations will be considered in accordance with applicable laws.
- Assist with intakes and implement personalized care plans.
- Manage documentation and maintain confidentiality.
- Understand issues surrounding chronic homelessness and medical care.
- Coordinate medical, mental health, and drug/alcohol appointments.
- Maintain professional relationships and monitor client behavior.
- Ensure a safe environment and make emergency calls as necessary.
- Communicate daily with clients and shelter staff.
- Assist clients in transitioning out of the Recuperative Care program.
- Complete required logs, reports, and client documents.
- Coordinate housekeeping and distribute hygiene items.
- Oversee food service and maintain sanitation practices.
- Interact with service providers and train staff/volunteers.
- Provide transportation to necessary appointments.
- Assist clients with daily living activities.
- Attend staff meetings and training sessions.
- Adhere to safety rules and perform additional duties as needed.
