Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
- Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Head Start, and Child Development Center.
- Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.

Job Title: Outreach and Enrollment Specialist
Reports to: Customer Service and Enrollment Manager
Areas of Responsibility: Provide outreach to community members and information to existing CAHC patients and clients on enrollment in the health insurance Marketplace Exchanges, Medicaid, CHIP, NBHS, Economic Assistance & Medicare programs. Serve as an advocate and refer patients, as appropriate, to other agency programs and/or external community agencies as needed. Provide training and support to Front Desk Staff.
Major Duties:
- Educate individuals about the availability of the Marketplace Exchanges, Medicaid, CHIP, NBHS, Economic Assistance & Medicare programs.
- Assist uninsured individuals for eligibility in the Marketplace Exchanges, Nebraska Medicaid, CHIP, NBHS, Economic Assistance & Medicare programs, and assist them in completing the appropriate application.
- Provide application assistance and facilitate enrollment of eligible patients and community members in health insurance programs Marketplace Exchange, Medicaid, CHIP and Medicare.
- Provide enrollment education via presentations and coordinate with other CAHC programs and outside agencies to recruit potential enrollees. Initiate contact with uninsured individuals through outreach activities to community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues.
- Maintain knowledge and expertise in eligibility, enrollment, and program specifications of the Federal Marketplace and other health coverage programs such as Medicaid, CHIP and Medicare.
- Work to continually build new relationships and maintain existing ones with current and future community entities that are providing services to consumers to promote health insurance coverage through the Federal Marketplace, Medicaid, CHIP and Medicare.
- Verify insurance eligibility as needed.
- Conduct New Patient Orientation with individuals who are new clinic patients in Primary Care and Behavioral Health. Determine patient eligibility for agency programs and income eligibility for payment and billing. Collect and verify insurance and other documentation to be included in patient profile history. Update and maintain current patient history as needed.
- Assess CAHC patients for social determinants of health needs and provide referrals to internal and external resources for patients/clients as needed.
- Maintain CPR (Cardiopulmonary Resuscitation) Certification.
- Possess a working knowledge of agency sliding fee scales.
- Follow up with potential enrollees and applicants to ensure program enrollment.
- Pull monthly reports regarding application and enrollment statistics.
- Travel throughout the service area to provide outreach to community members.
- Participate in regularly scheduled conference calls, meetings, and training to provide accurate, quality services. This includes participation in the Centers for Medicare and Medicaid Services (CMS) trainings and other required training to complete and maintain Certified Application Counselor certification and to enhance outreach and enrollment expertise.
- Ensure the protection, data-sharing consent, and security of a consumer’s personal, confidential, and identifiable information in a professional and responsible manner and carry out all measures to prevent unauthorized disclosures.
- Maintain confidentiality of records and communications following CAPWN policy and HIPAA privacy and security policies and procedures.

Working Conditions:
- Work must be completed with minimal supervision and requires a self-motivated individual.

- Occasionally travel around the panhandle throughout all weather seasons.
Physical Requirements:

- Occasional lifting and carrying of up to 25 pounds.
- Must regularly talk, hear, and have close vision ability.
Minimum Qualifications:
Grade: 2
- Associate degree OR minimum of one to two (1-2) years’ experience performing intake services, or customer service tasks preferred.
- Preferred bilingual skillset with incentive commensurate with skill and ability to speak, read and write in Spanish.
- Possess strong written and oral communication skills.
- Valid driver’s license with good driving record and insured vehicle available at all times.
- Ability to work cooperatively and collaboratively with all levels of management and external agencies to maximize performance, creativity, problem solving and results.
- Ability to apply knowledge of a variety of federal, state, and local resources, programs and services involved with Outreach and Enrollment initiatives.
- Ability to use intermediate functions and features Internet-based navigation and MS Office Suite including word processing, spreadsheets, presentation, and database applications.
- Highly organized and efficient at completing and maintaining written and computerized patient records.
- Ability to work in a team environment.
Grade: 3
- Meet the minimum qualifications above AND
- Have three to four (3-4) years of experience performing intake services, or customer service tasks OR
- Possess experience with the Federal Marketplace and/or other health coverage programs such as CHIP, Medicaid and Medicare OR
- Have experience working with the public explaining program benefits.
Grade: 4
- Meet the minimum qualifications above.
- Have five (5) or more years of experience performing intake services, or customer service tasks OR
- Have extensive knowledge about the Federal Marketplace and/or other health coverage programs such as CHIP, Medicaid and Medicare.
