This position responsibilities include maintaining a caseload of economically disadvantaged individuals working to improve their socio-economic situation through the use of case management principles and procedures.
Primary Duties and Responsibilities:
- Provide support to case management staff and supervise them in the supervisor's absence.
- Assist with supervision and monitoring the delivery of case management services including appropriate referrals, client progress, attainment of goals, and HMIS file management.
- Assist in developing ongoing intensive case management plans and monitor client progress as needed.
- Manage a caseload of (5-10) economically disadvantaged individuals
- Review assessments and social history, monitoring the development of a case management plan and individual self-sufficiency goals for each client.
- Monitor and support client progress through regular contact and follow-up.
- Instruct clients in basic living skills, including budgeting, comparative shopping, health/hygiene, nutrition, parenting, self esteem building, communication, group facilitation and other topics as needed or requested.
- Provide effective and timely documentation of client intake, assessments, program eligibility, case notes, goal planning, client progress and contacts through the HMIS system and ensure the accurate maintenance of client files.
- Periodically run HMIS reports.
- Develop a network of social services, refer clients to appropriate resources, provide advocacy and support, and share this network with staff.
- Identify, document, report, and brainstorm creative ways to overcome systemic barriers to client self-sufficiency.

Position Objectives:
- Maintain and expand existing programs to provide the best possible services to clients.
- Maintain order and structure within the case management staff to ensure compliance and continued funding of programs.

Qualifications:
This position requires a minimum of a Bachelor’s Degree from an accredited college or university. A Master’s Degree in Social Work or a related field, and LSW (Licensed Social Worker) is preferable. Requires a minimum of three years of progressively responsible experience in the human services field.
The person in this position must demonstrate outstanding organizational skills and the ability to manage multiple priorities and concerns. This position requires an individual with outstanding clinical skills and strong supervisory capabilities. The individual must have excellent communication skills, both oral and written, combined with strong interpersonal skills. The ability to work well with people from different cultural and socio-economic backgrounds is crucial to this position.

While performing the duties described herein, the employee is regularly required to talk, hear, stand, walk, maneuver stairs, sit, stoop, kneel, crouch, and crawl. The employee must occasionally lift and/or move up to 15 pounds. Specific vision capabilities required by this position include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
