What are the responsibilities and job description for the Floor Instructor position at Communities of Don Guanella and Divine Providence?
Essential Job Functions
· Will teach and assist clients in completing tasks and activities.
· Will implement program and goals for each individual.
· Will plan, organize, and implement community outings and related daily activities.
· Will participate in annual program plan reviews for assigned individuals in the program.
· Will focus daily planning and implementation of activities according to each individual’s identified strengths, needs and desires as identified on his/her program plan.
· Will complete all necessary documentation on a consistent basis.
· Will be responsible for the daily care, supervision, safety and training of assigned individuals.
· Will participate in ongoing in-service training and participate in at least 24 hour of in-service training annually including , but not limited to First Aid Training, personal emergency interventions, Fire Safety and other safety techniques
· Will be responsible for proper implementation of de-escalation techniques and authorized restraint techniques in accordance with the emergency restraint policy
· Will respect each individual’s limitations and assist each individual to make choices.
· Will promote good social interactions with each individual that is served.
· Will provide each person with as much assistance as necessary to complete tasks.
· Will communicate with the Director, Assistant Director , Coordinator of Day Programs, Activities Coordinator and Program Specialists/Q.M.R.P.’s about concerns arising in the care and training of assigned individuals.
· Will take an active interest in implementing, prompting, and protecting the civil rights of each individual.
· Will be able to perform a one person pivot as well as assisting individuals to bear weight and ambulate with or without assistance.
· Will demonstrate ability to perform a two person lift and transfer of individuals to and from their wheelchairs.
· Will maintain confidentiality relative to individual’s records, training progress, tracking documentation according to HIPPA regulations.
· Will promote a safe and clean working environment.
· Will be responsible for maintaining a positive work atmosphere by acting(behaving) in a professional manner and promoting camaraderie with co-workers, supervisors and others.
· Will share responsibilities in monitoring breaks, lunches and van duty.
· Will be responsible for consistent punctuality, attendance and maintenance of a proper appearance..
· Will know and follow all Developmental Programs Divisions policies and procedures.
· Will perform all other duties as directed by the Administrator, ATF Director, and ATF Assistant Director.

Qualifications:

· At least one year’s prior experience working with individuals with Developmental Disabilities.
· Must be able to be authorized and maintain authorization to drive company vehicles and or residents per the guidelines with the Archdiocesan Office for Insurance Services
Education/Experience:
- High School Diploma or equivalent is required.
Language Ability:
Must be able to effectively understand, read, write, print legibly, speak and do the following using the English language: read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals; write routine reports and correspondence.
Reasoning Ability:
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form; ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills:
To perform this job successfully, an individual should have intermediate knowledge of Microsoft Word, Excel, Internet Explorer, and Outlook.

Supervisory Responsibilities:
This position is not responsible for supervision of staff.
Work Environment:
- Fixed work schedule
· While performing the duties of this job, the employee will be required to in a variety of environments and settings with minimal exposure risk to adverse weather conditions.
· The noise level in the work environment is usually moderate. Must be able to handle various interruptions throughout the work day.
Physical Demands:
· The employee must regularly lift and or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift with assistance and/or move up to 50 pounds including lifting and carrying of individuals with physical disabilities
· Specific vision abilities required by this job include Close vision, Distance vision, Peripheral vision, Depth perception and Ability to adjust focus.
· The employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms and talk or hear.
· The employee is frequently required to stand; walk and sit and is occasionally required to climb or balance and stoop, kneel, crouch, or crawl.

Pay starts at $18.25/hr. for experience. Full-time positions are eligible for comprehensive benefits to include but not limited to medical, dental, vision, paid time off, retirement, etc.
