Communities In Schools of North Central Washington (CISNCW), in partnership with 13 other statewide network affiliates (11 independent 501(c)(3) organizations and 2 emerging organizations) across Washington state has been striving for equity in educational opportunities. Our office is a small, high-energy office that has a history of strong operations and successful outcomes built upon community partnerships. Driven by our core values of belonging, integrity, and relationship building, and in adherence to the CIS National guidelines, our North Central Washington office works to ensure that all educational communities in our area have the tools and capacity to provide and sustain strong student programs and services in their communities as we work directly inside K-12 schools supporting students who have historically been furthest from educational justice.

Position Summary

The Community Health Care Coordinator is responsible for delivering comprehensive care coordination services to clients across a large, primarily rural service region. This position is combining office-based administrative work with field visits to clients’ homes or other appropriate locations.

Under the Care Coordination Organization (CCO) program, administered through Community Choice, the coordinator supports clients in achieving their health and well-being goals by developing person-centered care plans and facilitating access to physical, behavioral, and social services.

This role requires a compassionate, organized, and self-directed professional with strong communication skills and the ability to build trusted relationships with clients and community partners.

Key Responsibilities

  • Conduct outreach and engagement with assigned clients via phone, email, mail, and in-person visits.
  • Establish supportive, trust-based relationships with clients, families, and caregivers.
  • Complete health screenings and assessments to determine client needs and eligibility.
  • Collaboratively develop Health Action Plans (HAP) with clients to identify and work toward both short- and long-term goals.
  • Promote client self-management and empowerment using a strengths-based approach.
  • Provide services in a respectful, trauma-informed, and culturally responsive manner that acknowledges social determinants of health.
  • Coordinate care with medical providers, behavioral health services, and social support organizations.
  • Maintain accurate, timely documentation in Electronic Health Records (EHR) and health information systems.
  • Adhere to HIPAA and organizational confidentiality policies for all client information.
  • Respond to client inquiries and concerns in accordance with program policies and procedures.
  • Represent CISNCW and Community Choice professionally when working with clients and community partners.
  • Support program-specific assignments or projects as needed in collaboration with leadership.

Qualifications

  • Education: Associate degree in Nursing, Social Work, Psychology, Public Health, or related field required; Bachelor’s degree preferred.
  • Experience: 2–4 years of experience in care coordination, case management, or related healthcare/social service roles. Home health or community-based experience strongly preferred.
  • Language Skills: Bilingual (Spanish/English) strongly preferred.
  • Technology Skills: Proficiency with EHR systems and Microsoft Office (Word, Excel).
  • Other Requirements:
  • Strong knowledge of local community resources and systems of care.
  • Reliable transportation and ability to travel up to 50 miles for client visits several times per month.
  • Ability to work independently, manage time effectively, and maintain organized records.
  • Must pass a criminal background check.

Working Conditions and Physical Requirements

  • Hybrid work with required travel to client homes and community sites.
  • Flexible schedule with ability to work evenings and/or late afternoons preferred.
  • Must be able to sit or stand for extended periods.
  • Ability to hear, see, and speak clearly for client and team communications.
  • Must have a valid driver’s license and current auto insurance.
  • Ability to lift up to 25 pounds.
