Company Overview:
Communities for People (CFP) is a non-profit agency serving youth and families through community-based programs and services. We are looking for a Clinician for a residential Program. You will be instrumental in helping youth and families to achieve permanency, independence and integration into the community. Group Home Summary:
Oversee the treatment planning and clinical case management needs for the youth in the program.
Group Home Clinician Responsibilities:
- Provide residents with direct clinical services, including case management, individual treatment, group work or clinically appropriate services
- Complete all required clinical documentation including clinical assessments, Relapse Prevention Plans, and Individual Treatment Plans
- Manage referrals for placement at the program and oversee the intake process
- Determine appropriate transition and discharge plans for all residents
- Assist program staff to develop individualized behavior management plans and support staff to understand and respond to individual youth treatment concerns
- Facilitate case conferences and discharge planning for all residents
- Assist with family engagement, permanency planning and life-skills development for all residents.
- Oversee coordination of community-based psychiatric, psychological, medical and dental services for all clients
- Actively participate in the Program Administrative Team, weekly staff meetings and group supervision
- Participate in agency activities to support the implementation of clinical best practices and improve the clinical skills of agency staff
- Be a part of the clinical on-call rotation
Group Home Clinician Qualifications:
- Master's Degree in social work or related field
- At least 3 years of clinical experience
- Prior familiarity with the Massachusetts child welfare systems,
- Must have valid driver’s license and vehicle
Job Type: Full-time
Pay: $65,000.00 - $73,000.00 per year
