What are the responsibilities and job description for the Patient Services Float *Gas Stipend Provided position at CommuniHealth Services, Inc.?
The Patient Services Float is responsible for establishing and maintaining a professional relationship with patients, visitors, and staff. The Patient Services Float is responsible for basic communication and relevant tasks related to the Patient Services Department. The Patient Services Float collaborates with staff to facilitate and coordinate clinical needs of patients. This position requires compliance with CornrnuniHealth Services (CHS) written standards, including the Compliance Plan and policies and procedures (including the Code of Conduct).
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Maintains a professional appearance while interacting with patients, visitors, and staff.
2. Maintains working knowledge of all relevant electronic platfmms and practice management systems.
3. Maintains knowledge of and compliance with Patient Services policies and procedures.
4. Collaborate with fellow Patient Services staff in a teamwork environment.
5. Counsel's patients upon first visit to the clinic, with a face-to-face encounter, regarding all available services at CHS and enter date into the Medical Home Information field in Athena Quick view
6. Greets patients, visitors and staff with dignity and respect whether in person or on the phone according to CornrnuniHealth Services' standards.
7. Schedules medical, behavioral health and dental appointments electronically according to scheduling policy/procedures.
8. Reviews and works tickler list each day and schedules patients accordingly.
9. Enters accurate and complete data entry of information into CHS practice management system as required for UDS and PCMH.
10. Maintains patient confidentiality according to HIP AA laws and CHS policies and procedures as well as compliance with HRSA regulations.
11. Receives and sorts faxes electronically and submits electronically to the appropriate person or department.
12. Scans and indexes medical record documents into the Electronic Medical Record System.
13. Accurately collects and posts co-pays/deductible/co-insurance to patient accounts.
14. Completes a daily deposit and transports deposit to the bank according to accounting policy.
15. Maintains a tidy waiting area and work area visible to the public.
16. Requests patient's medical records from other entities, when appropriate, to ensure continuity of care.
17. Maintains knowledge of and compliance with the Sliding Fee Program and income requirements. Offers and assists all patients in completing sliding fee applications.
18. Acts as a patient advocate, identifies and develops new community resources and assists with problem solving techniques. Documents actions in a patient case or on SDOH referral.
19. Works with patients to identify government programs such as Medicaid, WIC, welfare, food stamps, social security disability or unemployment eligibility and documents actions in a patient case or SDOH referral.
20. Obtains and maintains certification as certified application counselor.
21. Assist patients with completion of online applications for health insurance or Medicaid.
22. Provides outgoing referral appointments to appropriate providers, per policy guidelines, and monitors activities to ensure services are being delivered, meets the needs of patients, and ensures the delivery of services provided meets policy requirements and standardization.
23. Follows up on referrals and orders per policy guidelines.
24. Maintains knowledge of and access to manual forms and websites for prior authorizations (PA) for high tech imaging, medical and behavioral health services.
25. Completes prior authorizations on high tech imaging, per guidelines, for providers while being the liaison between provider, insurance, and the patient.
26. Follows up with patients transitioning from hospital or emergency room discharge (transition of care TOC), schedules and appointment, request records for provider review.
27. Downloads pre-visit planning report from Azara, reviews patient charts for accuracy of included information and addition of missing information. Make notes as appropriate and distribute copies to each team member while hosting the morning huddle.
28. Provides transportation to and from appointments at CommuniHealth clinics, initial referral appointments scheduled by ConununiHealth, and 340B Pharmacies.
29. Obtains transportation usage agreement from patients utilizing CormnuniHealth transportation services.
30. Maintains the safety of patients and drivers at all times.
31. Ensure all passengers are secured in a seatbelt and/or an age-appropriate child safety seat at all times.
32. Operates vehicle using standard operating procedures.
33. Completes required CHS training per policy, maintains compliance with and knowledge of all CHS departmental, facility specific and system-wide policy and procedure, attends mandatory departmental meetings and/or training, and follows CHS Mission, Vision and Values in delive1y of all services.
34. Participates in all appropriate meetings within the organization.
35. Performs other duties as may be assigned.
EDUCATION:
High School Diploma or GED
SKILLS/EXPERIENCE:
1. Must be able to read, write, speak and interact in the English language.
2. Must have good verbal and written communication skills.
3. Must maintain a professional personal presentation.
4. Must possess excellent customer service skills.
5. Must be attentive to detail
6. Must be reliable and trustworthy.
