About the Role:
In this role, you will support parents as they navigate local systems and their own family dynamics, helping them access resources, build confidence, and take the lead in their plan of care. In this community‑based role, you’ll mentor families, advocate on their behalf, and work with a dedicated team to help them thrive.
What You’ll Do
- Partner with families and support teams to develop strength‑based, family‑driven plans of care
- Help families access community resources across the Greater New Haven area
- Participate in child and family team meetings and ensure ongoing engagement
- Advocate for families across various systems, providers, and service settings
- Mentor, educate, and empower parents to take an active leadership role in their own care plans
- Collaborate closely with the Family Support Specialist and other program staff
- Transport clients in your own vehicle when needed

Qualifications:
- High school diploma with at least three years’ experience working with families.
- Bi-lingual Spanish-English preferred.
- Knowledge of community resources and services.
- Must be available to work evenings and weekends to meet with families as needed.
- Ability to work independently and as part of a team
- Sensitivity to diverse cultures and language competencies.
- Strong communication and documentation skills.
- Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
- Ability to prioritize and organize schedules/demands.
- Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
Salary : $19 - $27
