POSITION SUMMARY: The case manager is responsible for providing person centered case management services that promote the health, safety, welfare, independence, and overall well-being of individuals receiving waiver services. This role focuses on empowering participants to live inclusive, safe, and fulfilling lives within their communities. Case management involves direct collaboration with participants, their families and identified supports to develop, implement, and monitor person centered service plans. Case managers assist individuals in gaining access to needed Medicaid waiver-funded and non-Medicaid services, (i.e., social, medical, educational, vocational) and are responsible for implementing guidelines for Medicaid Waiver programs and Participant Directed Services across the region. The Case Manager ensures the implementation and oversight of individualized Person-Centered Service Plans (PCSPs) while promoting participant choice, satisfaction, and wellbeing in a conflict-free service environment.

REQUIRED SKILLS/ABILITIES: Possesses superior organizational, oral, and written communication skills. Must demonstrate a high level of familiarity with Microsoft Office and exceptional customer service skills. Must be dependable and have a strong sense of commitment. Able to interact effectively with the public and staff, able to adapt to and deal with frequent change, delays, or unexpected events.

PHYSICAL AND ENVIRONMENTAL DEMANDS: Work is physically light, standing and sitting, occasional stooping. Combination of office-based, community-based, and remote work settings. Flexibility in scheduling. Frequent talking and active listening when communicating with individuals, families, guardians, staff, and other service providers. Visual acuity and manual dexterity needed for required paperwork and data entry. Visual acuity necessary for appraising the appearance of individuals and their environment. Must be available to conduct in-person visits monthly and respond to participant needs in a timely manner. Monthly visits in the home with individuals may cause exposure to communicable disease, blood borne pathogens, unsanitary conditions, abuse, and violence. Ability to handle stress effectively and make decisions accordingly. May require evening or weekend consultation.

SPECIFIC FUNCTIONS/TASKS/RESPONSIBILIITES:

  • Monthly face to face Home Visits to monitor services, goals and health, safety welfare.

B. Facilitate, review and update client service plans.


  • Monitor Health, Safety and Welfare. APS/CPS/Incident Reporting as necessary.

D. Performs other duties or responsibilities as needed or assigned.


E. Meet productivity standards and deadlines for note submissions, reassessments, and plans of care:


1. Face-To-Face visit notes uploaded into the MWMA system within 10 calendar days of the face-to-face visit.
2. All non-billable Monthly Summaries uploaded into the MWMA system within 10 calendar days of the month following the month the note was for.
3. MAP 351 assessments uploaded into MWMA prior to 40 days before current Level of Care expires.
4. Plan of Care recertification submitted in MWMA prior to 7 days before current Level of Care expires.
5. All Plan of Care paperwork sent to Medical Records to be uploaded into the Credible EHR system within 90 days of new Level of Care.

QUALIFICATIONS:

1. Bachelor’s degree in social work, Human Services, Psychology, Nursing, or a related field (required). Prior experience in case management, direct support services or prior work experience with IDD population preferred.

2. Knowledge of person-centered planning principles and community-based services and resources.

3. Strong interpersonal, communication and problem-solving skills.

4. Ability to work independently with minimal supervision.

5. Must have reliable transportation, ability to travel as needed and to maintain a valid driver’s license and minimum state required automobile insurance.

6. Must pass all required background checks and drug screen and meet all state and agency training requirements.

MANDATORY REQUIREMENTS:

6. Adheres to all policies and procedures of Inlet Health and subsidiaries.

7. Attends mandatory in-services and other training/educational programs as required.

Benefits:

Benefits become effective on the first of the month following 60 days of employment. Medical, Dental, Vision, Health Savings Account, Flexible Spending Plan, Life Insurance, Short- and Long-Term Disability, Critical Illness, Accident Insurance, and Hospital Indemnity Insurance.


Full-time employees receive 12 days of vacation time and 12 days of sick time accrued throughout their first year of employment. Employees receive 10 holidays per year with two of those being floating holidays that can be used as they need them. We also provide up to four hours of employee appreciation hours per year and a pop-up store to select and item from.


Employee Assistance Counseling services available to all employees with limited dependent benefits. Full-time employees are eligible for up to 10 total visits per calendar year; dependents are eligible for up to 6 visits. Half-time employees are eligible for up to 6 visits; dependents are eligible for 3 visits. PT and PRN employees are eligible for 3 visits; dependents are eligible for 3 visits.

Salary : $48,673 - $63,113
