- The provider must be an experienced, trained beautician licensed in the State of Virginia.
- They must have liability insurance in the amount of $1M per occurrence, $2M aggregate, and will provide the community with a Certificate of Insurance on an annual basis.
- The provider has worker's compensation insurance per state-required limits and will provide community with certificate of insurance on an annual basis.
