EXAMPLE OF DUTIES
This position supports the Division’s mission to promote healthy lifestyles, prevent injury and disease, and assure the safe delivery of quality health care for all people in the Commonwealth. In this role you will assess, plan, administer, implement, and evaluate Diabetes Self-Management Education and Support initiatives by performing the following duties:
Strategic Coordination: Coordinate the planning, implementation, and evaluation of goals and objectives for diabetes self-management and support strategies and initiatives; facilitate meetings; and prepare and conduct presentations- Technical Assistance: Provide direction in the development and implementation of data collection, surveillance, and intervention activities and assistance on quality improvement in health systems using electronic health records
- Compliance Monitoring: Ensure program goals and policies are being met by using status calls, monitoring reports, site visits, and other relevant monitoring and evaluation tools
- Program Evaluation: Analyze public health programs, evidence-based interventions, and the improvement of health outcomes for people with diabetes using qualitative and quantitative data in collaboration with an evaluation partner
- Contract Management: Create contracts, grants, work statements, program narratives, program budgets, budget projections, and spending plans
- Partnership Building: Collaborate with national, state, and local organizations to establish policies and programs to improve the health of people with diabetes

Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 5:00 PM, Monday - Friday, with a 60-minute lunch.
- Telework: You may have the opportunity to work from home (telework) part-time. This position currently reports to the office on Tuesday and Wednesday each week. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Harrisburg. The ability to telework is subject to change at any time. Additional details may be provided during the interview.
