Aging Care Manager 2 (Local Government) - Wayne County Area Agency on Aging

As an Aging Care Manager 2, you will work with older adults from the initial contact with individuals, their families, and professionals through ongoing care management responsibilities. You will maintain a caseload of consumers providing various necessary areas of assistance. Incumbents regularly coordinate services with physicians, community agencies, and financial institutions to aid individuals. In this role, you will demonstrate your attention to detail and multitasking skills to complete paperwork, work assignments, and meet deadlines. This role includes specialized duties such as Protective Services, Care Transitions, and Person-Center Counseling.

Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
• This position has on-call hours when required.
• This position has overnight travel or out of county travel for meetings and training.
• Telework: You will not have the option to telework in this position.
• Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

Legal Requirements:

• You must pass a background investigation.
• A conditional offer of employment will require a drug screening.
• This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
