What are the responsibilities and job description for the Registered Nurse – Hospice On-Call | Flexible Schedules position at Commonwealth Hospice?
We're looking for a compassionate and dependable RN to join our growing hospice team. This is a field-based on-call position serving patients throughout the Pittsburgh and surrounding areas.
Flexible Scheduling Options:
- Weekend Coverage: Friday at 4:00 PM through Monday at 8:00 AM.
- Weekday Night Coverage: Monday through Thursday, 4:00 PM to 8:00 AM the following morning.
- 7 Days On / 7 Days Off: A rotating schedule that includes the applicable weekday and weekend coverage time frames.
- Schedules may vary based on coverage needs.
The primary coverage area is Pittsburgh and surrounding areas.Responsibilities
- Provide compassionate hospice care to patients and families in their homes and facilities
- Assess patient needs and respond appropriately to changes in condition
- Manage and monitor pain and other symptoms
- Respond to patient and family calls and provide on-call support
- Make visits as needed during on-call hours
- Document patient condition, visits, and care provided accurately and timely
- Respond appropriately to medical and end-of-life emergencies
- Collaborate with the interdisciplinary hospice team
- Follow all applicable hospice, nursing, and regulatory standards
- Provide education and support to patients, families, and caregivers
- Communicate effectively with physicians, nurses, social workers, chaplains, and other members of the care team
What We're Looking For
- Current RN license in Pennsylvania
- Valid driver's license and reliable transportation
- Strong clinical assessment and communication skills
- Compassionate, dependable, and comfortable working independently
- Ability to respond to patient needs during scheduled on-call hours
- Hospice experience is preferred but not required
