- Provide skilled care to our patients allowing them to return to independence.
- Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
- Educate our patients and their families on the next steps in continuing care and improvement of day-to-day life.
- Coordinate and perform skilled treatments to enhance the well-being of the patient with the goal of enhancing patient health.
- Valid registered nursing license (required)
- 1 years of registered nursing experience
- Associate degree in nursing (required)
- Home health experience is a plus.
