Overview
We are seeking a dedicated and highly skilled Home Health Registered Nurse (RN) to provide compassionate, patient-centered care in a home health setting on a PRN basis for the Middle neck and Northern neck areas. Responsibilities
- Conduct thorough patient assessments to develop individualized care plans that address medical needs, including chronic disease management, post-operative recovery, and palliative care.
- Administer medications and treatments such as IV infusions, injections, wound suturing, and catheter care while maintaining aseptic technique.
- Perform complex procedures including dialysis management, ostomy care, tube feeding, and ventilator support in accordance with physician orders and safety protocols.
- Utilize advanced clinical skills to manage acute pain, infectious diseases, trauma cases, and behavioral health concerns within the home environment.
- Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and continuity of care across settings such as hospitals, nursing homes, assisted living facilities, and community clinics.
- Document all patient interactions accurately within electronic health records (Wellsky), ensuring compliance with HIPAA regulations.
- Educate patients and their families on medication administration, disease management strategies, safety precautions, and health coaching to promote independence and wellness.
- Respond promptly to urgent triage calls or emergent situations using critical care response procedures while prioritizing patient safety.
Experience
- Proven clinical experience as a registered nurse with a strong background in home health care or outpatient settings.
- Demonstrated expertise in managing complex medical conditions such as cardiac disorders, respiratory illnesses requiring ventilator support, infectious diseases, or trauma medicine.
- Experience working with diverse populations including pediatrics (neonatal resuscitation procedures), geriatrics (memory care & dementia), behavioral health patients, and individuals with developmental disabilities.
- Familiarity with advanced nursing procedures such as ICD coding (ICD-9/10), laboratory specimen collection/processing, phlebotomy, and diagnostic evaluation techniques.
- Prior experience utilizing EMR/EHR systems like Athenahealth or eClinicalWorks for documentation and patient record management.
- Knowledge of Workers' Compensation law related to occupational health cases is advantageous.
- Additional experience in critical care environments such as ICU or Level I/II trauma centers enhances clinical versatility.
- Certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) is required to ensure readiness for emergency response situations.
