Job Summary and Responsibilities
As a Licensed Practical Nurse (LPN), you will provide compassionate nursing care to our residents, under the direction of a physician or Registered Nurse.
Every day, you will assess resident needs and deliver essential physical or psychosocial care.
To be successful in this role, you will exhibit genuine compassion for patient well-being, an understanding of your critical role in ensuring their comfort, and the patience to explain procedures, answer questions, and maintain accurate documentation.
- Assesses the patient’s condition and nursing care needs, and reports complex care needs and/or changes in condition to the RN or Wound Care Provider when applicable.
- Provides direct care in accordance with the prescribed medical and nursing treatment plans under the instruction of the Primary RN, Wound Care provider.
- Implements and evaluates appropriate patient and family education per Plan of Care and under RN supervision.
- Functions as an advocate for the patient and his/her family by supplying and interpreting information, supporting decisions, and facilitating communication.
- Completes all visits and documentation within regulatory and Agency timeframes.
Shift: 7:30am - 4pm
Schedule: Monday - Friday
Job Requirements
Required
- Oregon LPN License
- AHA BLS within 30 days
- LPN Program
- Two (2) years LPN experience
Preferred
- Medical, Surgical, Home Health or Wound Care experience
