Job Summary and Responsibilities
- Coordinates care with other health care community providers.
- Promote patient retention by providing focus on getting patients to the right service with the right provider at the right time – one call resolution.
- Manage phone call activity to minimize any delays experienced by customers.
- Conduct intake for new referrals to quickly and accurately triage the needs of the caller.
Job Requirements
Preferred- Bachelors degree in Social Work or Human Services related field
- Acute care experience in social work or behavioral services
