Job Summary

Committed 2 Caring Home Care Agency LLC is seeking a dedicated and compassionate Qualified Mental Health Professional (QMHP) to to join our dynamic behavioral health team. In this vital role, you will provide comprehensive mental health services, leadership, oversight, and clinical support to our Peer Support Team. This role is ideal for a motivated mental health professional who enjoys supervision, quality assurance, and person-centered care coordination.

The QMHP will supervise 1–4 Peer Support staff, ensure compliance with service standards, including assessment, diagnosis, and treatment planning for diverse populations, oversee clinical documentation, including Person-Centered Plans (PCPs) and peer support records. Your expertise will help individuals and families navigate mental health challenges, foster resilience, and achieve their wellness goals. The ideal candidate will be energetic, proactive, and committed to delivering high-quality patient care through evidence-based practices and innovative therapeutic approaches.

Responsibilities

  • Provide direct supervision and support to 1–4 Peer Support Team members
  • Complete, review, and maintain Person-Centered Plans (PCPs) in accordance with state and agency requirements
  • Facilitate and participate in team meetings, including case staffing and care coordination
  • Conduct audits of Peer Support charts and files to ensure accuracy, completeness, and compliance
  • Ensure Peer Support services are delivered in alignment with individualized goals and recovery-focused principles
  • Provide guidance, coaching, and corrective feedback to Peer Support staff as needed.
  • Utilize motivational interviewing techniques to enhance client engagement and promote positive behavioral change.
  • Manage complex cases involving children, adolescents, adults, geriatrics, individuals with developmental disabilities or autism spectrum disorder (ASD), PTSD, grief issues, addiction concerns, gender issues counseling, and chronic pain management.
  • Maintain detailed medical records and documentation in compliance with HIPAA regulations; ensure accurate medical terminology usage and timely updates of patient care notes.
  • Collaborate with multidisciplinary teams including social workers, case managers, medical professionals, educators in special education settings, and research staff to optimize patient outcomes.
  • Participate in clinical research initiatives and data collection efforts to advance behavioral health practices.
  • Provide crisis management and behavior management support during emergencies or acute episodes; coordinate with emergency responders as needed.
  • Maintain compliance with Medicaid, state, and agency documentation standards

Requirements

  • Bachelor’s or Master’s degree in Human Services, Mental Health, Psychology, Social Work, Counseling, or a related field
  • Minimum 1–2 years of experience in mental health services (Peer Support or supervision experience preferred)
  • Experience with PCPs, documentation review, and chart audits strongly preferred
  • Knowledge of Peer Support services and recovery-oriented care
  • Strong organizational, communication, and leadership skills
  • Ability to supervise staff, manage multiple responsibilities, and meet deadlines
  • Proficiency in crisis intervention techniques and crisis management protocols; ability to perform patient assessment effectively under pressure.
  • Telehealth experience preferred; ability to adapt services for remote delivery while maintaining high standards of patient care.
  • Knowledge of HIPAA regulations; experience managing medical records securely and accurately.
