This role oversees the social function of the household, including activities. This position serves as a mentor/leader and partners with the Clinical Coordinator in the coordination of care of the residents within the household.
- Develops positive resident relationships, exploring approaches to aging and supporting families.
- Addresses and documents any special psychological concerns (death in family, health decline etc.).
- Maintains individualized psychosocial assessment.
- Evaluates personal and emotional concerns.
- Assists with the completion of an individualized activity assessment.
- Maintains household activity calendar and coordinates activities and ensure resident appointments are kept.
Requirements
The Preferred Candidate:
- Valid LPN Licensure (preferred)
- One (1) year experience in long term care, as an LPN (preferred).
- Must have critical thinking skills and decision-making ability.
- Must have excellent leadership skills and abilities.
- Excellent communication skills to effectively communicate with residents, families, and staff.
