Key Responsibilities
- Evaluate patient mobility and develop personalized care plans
- Provide skilled interventions to improve strength, balance, and functional independence
- Educate patients and caregivers on home exercise programs and fall prevention
- Supervise and guide PTAs as needed
- Monitor progress and update care plans accordingly
- Document visits accurately using Kinnser/WellSky EMR
Requirements
- Active Florida Physical Therapist license
- 1 year of experience in physical therapy (home health preferred)
- Strong interpersonal and clinical skills
- Reliable transportation and willingness to travel
- Familiarity with or willingness to learn Kinnser/WellSky EMR
- CPR certification
