Experience

Physical Therapist Assistant: 2 years preferred

Home health experience is a plus but not required
Key Responsibilities

Implement the PT‑developed plan of care to improve functional mobility, strength, and safety

Provide therapeutic exercises, gait training, balance training, and functional mobility interventions

Conduct home safety assessments and recommend modifications or equipment as appropriate
-
Monitor patient progress and communicate changes to the supervising Physical Therapist

Document all visits accurately and timely in the EMR
-
Collaborate with the PT and interdisciplinary team to ensure coordinated, patient‑centered care
-
Educate patients and caregivers on exercises, safety techniques, and home programs
