Job Summary
We are seeking a compassionate and highly skilled Hospice Visit Registered Nurse (RN) to provide comprehensive in-home and facility-based hospice care. The ideal candidate will have extensive experience in patient assessment, symptom management, and end-of-life care, ensuring comfort and dignity for patients facing advanced illness. This role involves coordinating with interdisciplinary teams, managing complex medical needs, and delivering personalized care plans aligned with hospice standards and regulations. The RN will utilize advanced clinical skills, including medication administration, IV infusion, wound care, and patient education, to support patients and their families through the end-of-life journey.
Responsibilities
- Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications, including injections and IV infusions, following aseptic techniques and safety protocols.
- Manage complex symptoms such as pain, dyspnea, nausea, and agitation with appropriate pharmacologic and non-pharmacologic interventions.
- Perform wound care, ostomy management, tube feeding, and catheter care as needed.
- Document patient care accurately using EMR/EHR systems such as Epic or Cerner while maintaining HIPAA compliance.
- Educate patients and families on disease processes, medication management, advanced directives, and comfort measures.
- Collaborate with interdisciplinary teams including physicians, social workers, chaplains, and therapists to develop comprehensive care plans aligned with hospice & palliative medicine standards.
- Coordinate discharge planning and facilitate transitions between inpatient facilities like ICU or trauma centers to home or long-term care settings.
- Participate in quality improvement initiatives adhering to NCQA standards and infection control protocols.
- Provide specialized care for diverse populations including geriatrics, pediatric patients (including neonatal ventilator management), individuals with developmental disabilities or dementia care needs.
Requirements
- Valid RN license with current state licensure; experience in hospice & palliative medicine preferred.
- Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD-10/ICD-9 coding is a plus.
- Demonstrated expertise in medication administration including injections, IV insertion, catheterization, tube feeding, and aseptic technique.
- Experience working in various settings such as hospital medicine, home & community care, outpatient clinics, or skilled nursing facilities like nursing homes or assisted living communities.
- Experience with hospice and HospiceMD
- Critical care experience including ICU or Level I/II trauma center exposure highly desirable.
- Knowledge of infectious disease care, pain management strategies, trauma medicine, post-acute care practices, and infection control procedures.
- Ability to perform physical assessments accurately; understanding of physiology and anatomy essential for diagnostic evaluation.
- Strong communication skills for teaching patients/families about health coaching and discharge planning; ability to work compassionately with people with disabilities or cognitive impairments such as Alzheimer’s or dementia.
- Must pass a background check and drug screening prior to employment; valid driver’s license required for travel between patient locations.
