Position Summary
The Administrative Assistant / Care Coordinator serves as a key member of our administrative team by coordinating patient admissions, scheduling clinicians and caregivers, managing referrals, maintaining accurate records, and supporting communication between patients, healthcare providers, hospitals, and referral sources.
The ideal candidate is detail-oriented, professional, customer-focused, and capable of managing multiple priorities while maintaining exceptional accuracy.
Key Responsibilities
- Process new patient admissions using Well Sky Home Health software.
- Enter and maintain accurate patient demographic, insurance, and clinical information.
- Coordinate patient care and monitor ongoing services.
- Review clinical documentation submitted by nurses, therapists, and other healthcare professionals for completeness.
- Answer and direct incoming calls from hospitals, physicians' offices, rehabilitation facilities, patients, and referral partners.
- Coordinate schedules for nurses, therapists, aides, and caregivers.
- Verify insurance eligibility, authorizations, and patient benefits.
- Maintain patient records in compliance with HIPAA and agency policies.
- Manage email correspondence, referrals, and electronic communications.
- Organize office files and maintain accurate administrative records.
- Assist management with reports, projects, and daily office operations.
- Maintain a professional, organized, and welcoming office environment.
- Perform additional administrative duties as assigned.
Qualifications
Required
- Minimum of 2 years of administrative office experience.
- Excellent verbal and written English communication skills.
- Strong organizational and multitasking abilities.
- Excellent attention to detail and accuracy.
- Strong problem-solving and decision-making skills.
- Proficiency in Microsoft 365 (Word, Excel, Outlook, Teams).
- Ability to learn and adapt quickly to new software systems.
- Professional telephone etiquette and customer service skills.
- Valid driver's license with a good driving record.
Preferred
- Previous home health, hospice, healthcare, or medical office experience.
- Experience using WellSky Home Health software.
- Knowledge of Medicare, Medicaid, and insurance verification processes.
- Bilingual (English/Spanish) is a plus.
