Job Description:
As a part of each participant’s care team, the SLP will work collaboratively to provide therapeutic interventions and support specific to the individualized needs, goals and abilities as defined by the plan of care. The SLP will provide direct service to participants, working on assessment and intervention to meet goals that will increase the individuals independence in ADLs and IADLs in the community. The SLP will be tasked with documentation and preparing all reports needed for regulatory monitoring maintaining the integrity of services provided to each participant.
Services provided
- Evaluate participants and develop treatment plans as determined necessary when supported by a physician ordered evaluation.
- Organize and implement therapy services to participants in home, community, and outpatient settings in accordance with the participants plan of care.
- Communicate with Comfortable Living administration to coordinate services and assure all documentation in place prior to service provision.
- Document all therapeutic progress notes in accordance with regulation within agency directed procedure, electronic records, and in compliance with medicaid standards.
- Provide detailed therapeutic notes including progress towards the goals and objectives identified in the person-centered service plan, the date of service, the beginning and end time and signature and title of the individual providing the service.
- Participate in participant team meetings as scheduled to collaborate on the treatment goals and supports.
- Complete all documentation required, in addition to progress notes, including but not limited to monthly summaries, annual summaries, and updated goals as requested by governing bodies.
- Provide assistance regulatory reviews as needed and assist with developing a plan of corrections as directly related to deficiencies in the delivery of Speech and Language pathology services.
Qualifications:
- A licensed speech and language pathologist
- Experience in working in an organization which serves an individual with a disability.
