What are the responsibilities and job description for the LPN Scheduling & Care Coordinator position at Comfort Keepers?
Under the general supervision of the Territory Manger, the Scheduler is responsible for supporting service delivery of Home Care services throughout Central Minnesota. This is a part time job during typical office hours, Office is in Buffalo. This position is responsible for both scheduling and Skilled Nurse Visits.

- Coordinate Service Delivery by matching caregivers to clients based on personality, skill, client's scheduling needs, locations, and caregivers' availability.
- Accurately staff clients with caregivers based on location, time, skills/needs, request, etc.
- Positive relationship with all team members contributing to a cohesive and engaging work culture with all staff and clients.
- Consistently handle caregiver and client's issues (i.e. time off requests, questions, changes in schedule and/or availability concerns).
- Effectively and accurately communicate with caregivers, clients, case managers, and office staff.
- Communicate staffing needs with the team.
- Answer incoming calls in a friendly, professional, and knowledgeable manner.
- Build plans and shifts to match insurance codes.
- Monitor electronic clocking in/out and late alerts. Track attendance and illness log.
- Demonstrate professional and effective communication
- Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.
- Must be have own reliable transportation to support outer offices as needed
- Pariticipate in on-call phone answering rotation
- Two-year degree or equivalent in training or work experience
- One year staffing/scheduling experience
- Medical Experience preferred
- Wage depending on education and experience
- Licensed Nursing LPN RN encouraged to apply
