- Coordinate care plans and caregiver schedules to ensure quality service
- Follow up with clients to maintain satisfaction and address concerns
- Collaborate with office staff to provide seamless care coordination
- Maintain accurate client records and required documentation
- Develop partnerships with healthcare providers and community organizations
- Support local networking events and community outreach initiatives
- Help identify opportunities to improve the client experience
- Excellent communication and relationship-building skills
- Strong organizational and time-management abilities
- Ability to prioritize multiple tasks in a fast-paced environment
