Job Summary
We are seeking a compassionate and dedicated Caregiver Home Care Aide to provide exceptional support and assistance to individuals in their homes or community settings. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity.
- Support patients with mobility and transfer methods using equipment such as Hoyer lifts and patient transfer techniques to promote safety.
- Monitor vital signs, observe patient conditions, and report changes promptly to healthcare professionals to ensure optimal care.
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation.
- Administer medication following care plans and ensure compliance with HIPAA regulations for patient privacy and confidentiality.
- Provide transportation assistance for appointments, errands, or community activities to foster independence and social engagement.
- Maintain detailed documentation of patient care activities, observations, and progress in accordance with facility policies.
Requirements
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Knowledge of dementia care, Alzheimer’s care, memory care, autism support, developmental disabilities, intellectual disabilities support, neurodevelopmental disorders, and geriatrics is advantageous.
- Ability to perform manual handling tasks including heavy lifting and patient transfers safely.
- Strong communication skills for effective client interaction and team collaboration.
- Experience working with diverse populations including seniors, individuals with disabilities, or those requiring hospice or palliative care.
- Demonstrated ability to observe patient conditions carefully and document findings accurately.
- Flexibility to work various shifts including evenings, weekends, or overnight as needed.
