What you will do:
- Support patients and families with reinstatement of insurance
- Collaborate with the Clinical Management Team to optimize patient outcomes
- Ensure timely and accurate submission of insurance applications
- Offer support to families as needs arise
- Bring comfort, clarity, and compassion to every patient interaction
- Master's in Social Work (MSW)
- Experience in home care or hospice preferred
- Strong communication and problem-solving skills
