Job Summary
The Community Health Coordinator (CHC) is part of the Community Health Team working to address health disparities through education and connection to resources and services. The CHC role will provide educational opportunities that build the confidence, skills, and knowledge of program participants and community members to stretch their food dollars, create nutritious meals with readily available ingredients, and introduce new ways to utilize nutritious foods in everyday life. Additionally, the CHC will support the department’s efforts to address the impacts of other health-related social needs on community food and nutrition security, and support access to services and programs in other sectors through resource navigation and outreach.
Responsibilities
- · Be familiar with Protected Health Information (PHI) and HIPPAA security protocols and demonstrate ability to adhere to these and other privacy policies.
- Maintain and log program data records on multiple platforms, and ensure that grant and contract target metrics are met within the budget provided.
- Develop educational curriculum and plan and execute cooking demonstrations and educational workshops at CFC food pantries and in collaboration with external partners.
- Follow food safety guidelines and obtain appropriate permits as needed.
- Develop and assemble healthy meal kits and recipes for distribution at the Comfort Food Community food pantries, and other community outreach events.
- Conduct formal and informal needs assessments with clients and provide resource connections or referrals based upon identified needs, from SNAP enrollment to transportation.
- Track client interactions utilizing a suite of case management tools with accuracy and timeliness.
- Outreach to partner agencies to spread awareness about our programs and stay informed about regional agencies, services, and referral platforms.
- Provide on-site support at the food pantries, including assisting with client needs, food movement, or helping run service as needed.
- Lend support to all other Food as Medicine program initiatives as needed, including tabling, tracking program metrics, and communicating with participating vendors.
- Supports program evaluation efforts within Community Health and across other organizational programming.
- Participate in community working groups and cohorts to maintain and advance relationships with partners.
- Maintain professional, responsible, and conscientious standards of care and confidentiality at all times.
- Participate in professional development webinars and trainings.
- Other duties as assigned.
Experience
- Bachelor’s degree preferred in public health, nutrition, education, social work or related field
- 1–3 years of experience in nutrition education, health promotion, care coordination, community outreach, or public health programming (includes internships).
- Must be a self-starter with the ability to take the lead on projects while coordinating and collaborating with the broader team’s activities and objectives.
- Familiarity with the community in which the work will take place and ability to utilize program resources within the community.
- Experience working with diverse communities, including individuals with limited access to nutritious foods.
- Familiarity with purchasing and preparing foods while staying within a budget.
- Experience with curriculum and recipe development, workshop facilitation, or culinary instruction is a strong plus.
- Knowledge of basic nutrition principles, chronic disease prevention, and healthy cooking techniques.
- Excellent communication and presentation skills, especially with non-expert audiences.
- Ability to develop engaging educational materials and hands-on learning experiences.
- Cultural competency and a strong commitment to health equity.
- Strong organizational skills and ability to manage multiple programs or events according to priorities developed with the supervisor.
- Comfort using digital tools (e.g., Canva, Teams, Microsoft Office, etc.).
- Spanish language proficiency preferred (not required).
Pay: $22.00 - $24.00 per hour
