Position Summary:
The Hospice Registered Nurse (RN) Case Manager is responsible for coordinating and managing the care of hospice patients and their families. This role focuses on providing compassionate, patient-centered care while ensuring quality, compliance, and interdisciplinary collaboration.
Key Responsibilities:
- Conduct comprehensive patient assessments (physical, psychosocial, emotional, and spiritual)
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (IDT)
- Coordinate care with physicians, social workers, chaplains, aides, and other team members
- Provide skilled nursing care, including pain and symptom management
- Educate patients and families about disease progression, medications, and end-of-life care
- Serve as the primary point of contact for patients and families
- Perform regular home visits and document care according to regulatory and agency standards
- Monitor patient condition and adjust care plans as needed
- Ensure compliance with Medicare, Medicaid, and hospice regulations
- Participate in IDT meetings and contribute to care planning discussions
- Provide emotional support and counseling to patients and families
- Coordinate discharge planning when appropriate

Pay: $85,000.00 - $90,000.00 per year
