What are the responsibilities and job description for the Community Living Support Worker position at Comfort Care Home Inc.?
Comfort Care Home has an exciting opportunity to join our team! We are looking to expand by hiring two part time staff in the Isanti County area. Staff can combine part time positions to work up to 40 hours a week.
The primary focus of this position is to work directly with people who receive supportive services in their own homes so that they can continue living independently. The direct care staff will act in the best interest of the person they serve by implementing personalized goals/outcomes and documenting process.
Direct care staff will be working with individual doing a variety of daily tasks like; recreational activities in the community and at home; assisting and training clients to increase skills and independence in daily living activities (dressing, bathing, medication administration, budgeting and other duties as assigned) We strive to have each employee work as a valued interdisciplinary team member with our staff as well as other professions, including; county case managers, medical needs personnel, therapy and psychiatric services.
The current need is for people served in the Isanti County area, specifically in the city of Cambridge and rural Isanti County
Experience is a plus, but we will train the right individual(s).
CURRENT AVAILABILITY:
**1 part time staff to work with a female on elderly waiver, in her home. This positions proposed hours would be Mondays and Tuesdays 10am-3pm and every or every other Sunday 10-3. This person would be responsible for ensuring IHS (in home supports without training) are met to ensure this person's health and safety needs are met, there is some homemaking, cooking/baking, socializing, shopping, running errands that are a part of this persons plan. This person does not require a need for 'personal cares' as she is independent in that area. No lifting other than groceries. Must be able to go up/down one flight of stairs several times a day. There are no pets in the home, this person does smoke cigarettes.
**1 or 2 part time staff to work respite time for his live in caregiver. This positions proposed hours would be Tuesday and Thursday 3pm-10pm and possibly every other Sunday 630pm-930pm. This position is in rural Isanti County. There are dogs inside the home and farm animals outside.
**1 on-call staff to work with individuals in their own homes. This position would be one that covers shifts for staffing vacations, sick call and training hours.
PERSON HIRED CAN WORK WITH MULTIPLE CLIENT LOCATIONS IF THEY CHOOSE, (up to 40 hours per week)
Qualifications
- Experience in direct support roles that promote an individuals desire to remain living in their own private home/apartment in the community. This position is not in a group home setting.
- Must be able to pass a background check
- Experience and training working with 245D is a plus
- Ability to work independently and make reasonable judgement calls to follow policy and procedures.
- Having reliable vehicle or transportation to and from job site is a must. A valid drivers license and valid insurance is strongly recommended.
- Strong cooking, organizing and cleaning skills for maintaining a safe living environment
- Potential hire must be able to commit to consistent schedule(s) and are expected to arrive on time and stay until the end of scheduled shift(s)

Pay: $20.00 - $21.00 per hour
