What You’ll Do:
• Provide skilled nursing care based on RN/physician plan of care
• Administer medications, treatments, and wound care
• Monitor patient condition and report changes
• Maintain accurate and compliant documentation
• Support patient safety and infection control
Requirements:
• Active California LVN license
• CPR certification
• Strong communication and clinical judgment
• Reliable transportation
Preferred:
• 1 year of home health experience
