Job Description: The RN Care Manager comprehensively plans for care management of targeted patient population. Performs resource management, discharge planning, care facilitation, referral to other levels of care and related duties. Works collaboratively with the multidisciplinary team to facilitate achievement of desired treatment outcomes.
Qualifications:
- Graduate of an accredited school of professional nursing.
- Valid license to practice as a Registered Nurse in NC.
- Incumbent will be required to demonstrate competence in the performance of duties specific to all age groups of patients.
Preferred: BSN, MSN, Accredited Case Manager Certification.
Experience: Three years of experience in acute health care.
