Responsibilities
- Provide individualized nursing care per resident care plans, including medication administration, health monitoring, documentation, and supervision of nursing assistants.
- Maintain accurate medical records, process physician orders, manage documentation, and ensure confidentiality while supporting admission and discharge processes.
- Participate in developing and updating care plans, attend care conferences, and ensure clear, effective communication with residents, families, and the care team.
- Support team performance by assisting with training, guiding paraprofessional staff, contributing to evaluations, and fostering a positive, respectful work environment.
- Ensure safety and regulatory compliance by following infection control and emergency procedures, reporting incidents, participating in surveys and QAPI initiatives, and contributing to ongoing quality improvement.
Qualifications
- Must possess a clear unencumbered license from the State of Colorado as a Licensed Practical Nurse or Registered Nurse.
- Knowledge of the aging process, dementia, and disabilities highly required.
- Experience in the geriatric population in a long-term care facility, assisted living facility, or other related healthcare facility highly preferred.

Pay: $27.00 - $45.00 per hour
