Role Description This is a full-time, on-site role for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) located in Columbia, SC. The provider will perform comprehensive assessments, manage acute and chronic kidney disease, and follow patients with hypertension and related conditions in both clinic and hospital settings. Daily responsibilities include obtaining histories, performing physical examinations, ordering and interpreting diagnostic tests, adjusting medications, and collaborating with nephrologists on individualized care plans. The role may involve rounding on inpatient consults, participating in the management of dialysis and transplant patients, and coordinating care with other specialists and primary care teams. The provider will also educate patients and families on disease processes, treatment options, and lifestyle modifications, while documenting all encounters accurately and timely in the medical record.
Qualifications
- Active, unrestricted license (or eligibility) as a Nurse Practitioner or Physician Assistant in South Carolina, with appropriate national certification (e.g., FNP, AGNP, PA-C).
- Experience or strong interest in nephrology, hypertension, dialysis, or transplant medicine; prior inpatient or specialty practice experience preferred.
- Proficiency in conducting patient assessments, ordering and interpreting labs and imaging, and developing evidence-based treatment plans.
- Strong clinical judgment, critical thinking skills, and comfort managing complex, chronically ill adult patients.
- Effective communication and interpersonal skills for working collaboratively with physicians, staff, patients, and families.
- Ability to work independently, manage time efficiently, and adapt to a fast-paced, multi-site clinical environment.
- Comfort with electronic health records and basic digital tools for documentation, ordering, and care coordination.
- Graduate of an accredited NP or PA program; current BLS certification required, ACLS and nephrology-specific training or certification a plus.
