What are the responsibilities and job description for the Clinical Social Worker - LCSW position at Columbia Memorial Hospital?
The Clinical Social Worker position provides biopsychosocial assessments/interventions, crisis assessment/interventions, care coordination, referral coordination, and advocacy for patients and their support system(s) as a member of the interdisciplinary team for Lower Columbia Hospice & Palliative Care. This position oversees the bereavement program to support loved ones of hospice patients as well as providing grief support to the community through education and support groups, often in partnership with trained volunteers.

Knowledge Of/Skill/Ability To
Maintains the standards of Social Work excellence in accordance with principles set forth by the State Health department, Columbia Memorial Hospital, and agency policies and procedures. Performs short-term crisis intervention and individual or family counseling when indicated.
License/Certification
Master’s in social work from CSWE accredited school. Minimum of one-year post-Masters experience in medical social work is required. Experience in hospice social work is strongly preferred. Competency in providing cognitive and behavioral assessments and interventions to individuals, families, couples, and groups with behavioral health and/or substance use complications in a health care setting.
Physical Demands and Work Environment Conditions / ADA Compliance
Involves standing, walking, stooping, and occasional carrying/lifting of lightweight materials. May be required to sit or stand for extended periods of time. Requires visual acuity and depth perception to recognize words/numbers Requires hand/finger dexterity and hand/eye coordination to use a computer keyboard and common office equipment.

- An Oregon Licensed Clinical Social Worker (LCSW) license is required, or ability to transfer licensure from another state with 60 days of hire.
- Ability to maintain credentialing and appropriate documentation for insurance companies, if needed.
- Valid driver’s license with current vehicle insurance coverage and safe driving record is required.
