Shift: Day

Location: 71 Prospect Ave, Hudson, NY 12534

Position Summary: The case manager works in collaboration with physicians, patients/families, other Health Professionals/organizations and payers to promote healthcare services that include support, education, efficiency, timeliness, and effectiveness while continuously working within the mission statement of Columbia Memorial Hospital.

Qualifications:

Education: Graduate from an accredited school of nursing.

Demonstrates an understanding of the NYS Practice Act.

License/Certifications: Registered nurse licensed to practice in NYS.

Certification in Case Management preferred.

PRI/certification preferred.

Experience: Previous experience in case management preferred.

Minimum of three (3) years experience as a licensed, registered nurse, preferable five (5) years.

General computer knowledge and capability to use computers required. Critical thinking skills and ability to analyze complex data sets required.

Responsible to: Director of Social Work Services/Case Management

Responsible for: Acute Care patients, Emergency Department Patients and Care Center Medical Home Care Management
