Level 1 (Entry level up to 2 years of direct experience): $25.24 per hour

Level II (3 years of direct experience):  $26.43 per hour

Bilingual premium for qualifying languages is 3.5% higher than starting salary if pass bilingual exam on first week of hire.

Health Access Program at CHN: The Health Access team at CHN contracts with the Colorado Department of Public Health & Environment (CDPHE)’s office of STI/HIV/VH to carry out certain aspects of the State Drug Assistance Program (SDAP). This program prioritizes improving access to health care for individuals living with HIV who qualify for SDAP program benefits. The statewide Health Access team works directly with clients to facilitate medical insurance enrollments, navigate challenges in the US Health Care System, and provide financial assistance for qualifying medical expenses.

Position Summary: The Health Access Aging Services Coordinator plays a key role in supporting older adults living with HIV by facilitating health insurance access and related healthcare services. This position maintains an active caseload of 100 clients over the age of 50 and provides one-on-one assistance with Medicare, Medicaid, private insurance enrollment, and Social Security Disability Insurance (SSDI) navigation. Due to the nature of health insurance navigation, this position has notable shifts in demand based on season, with General Open Enrollment Periods at the end of each year requiring the highest volume of work. The Coordinator works closely with CHN departments, community partners, and state agencies to ensure smooth client transitions and warm handoffs.

This role is ideal for a professional with a strong understanding of aging-related issues, HIV/AIDS, Medicare, and public health systems. The successful candidate will be organized, compassionate, and adept at helping clients overcome complex barriers to care.

Performance in this position is measured through formal supervision, electronic data analysis, and routine quality assurance auditing.


Essential Duties and Responsibilities

  • Educate clients and the public on SDAP and relevant updates to healthcare policy and access
  • Conduct enrollment appointments for clients over age 50, including support with Connect for Health Colorado, Medicaid, Medicare, and Medicare Savings Programs
  • Provide quarterly outreach and ongoing assistance to clients aging into Medicare
  • Develop individualized Medicare and SSDI transition plans
  • Help clients complete necessary paperwork for program participation
  • Coordinate and track invoice processing and bill payments for eligible clients
  • Work with pharmacy and clinic partners to troubleshoot medication access and insurance issues
  • Maintain accurate, timely data entry in electronic systems
  • Serve as a knowledgeable resource for clients on healthcare access and insurance navigation
  • Foster and maintain positive, professional relationships with clients, coworkers, and community partners
  • Participate in ongoing training and maintain current knowledge of systems related to Medicare, SSDI, and public benefits

Qualifications:

Qualification:

  • Knowledge of HIV/AIDS, health insurance systems, and aging-related issues
  • Experience assisting with healthcare navigation and benefit programs, including Medicare and Medicaid
  • Preferred: Previous experience as a Certified Application Counselor or Health Coverage Guide
  • Excellent organizational and time management skills
  • Demonstrated interpersonal and communication skills, both verbal and written
  • Ability to work independently and as part of a team
  • Commitment to culturally responsive and trauma-informed service delivery
  • Strong attention to detail and ability to maintain confidential information
  • Bilingual or multilingual candidates, especially Spanish-speaking, are strongly encouraged to apply


Education:
Bachelor's Degree in Social Work, Human Services, Public Health, Gerontology, and/or a related field or equivalent experience required.
Software Utilized:
Experience working with basic office and database computer programs, including familiarity with Microsoft Outlook, Word, Excel, Access, SharePoint and social media outlets required.

 

Knowledge, Skill and Ability:

Previous experience in the delivery of evidenced-based programs is highly preferred, as well as excellent communication, client services, and data collection skills.  The appropriate person will be highly organized and detail-oriented.  This position requires personal and professional accountability; self-management and initiative; dedication to quality improvement; professional integrity; research ethics, and cultural competency.  Must demonstrate accuracy, efficiency and superior organizational skills.  Must demonstrate effectiveness in the following areas: task and time management; teamwork; and communication with teams, managers, clients and participants, and company consultants.


Material and Equipment Directly Used:
Works with standard office equipment including computer workstation, printers, copiers, scanners and fax machine. Works with word processing software, spreadsheet applications, and presentation software. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax.
Physical Demands and Work Conditions:
Works in an office environment. Will travel to offsite locations. Regular weekly and monthly reimbursable travel required. Requires ability to lift up to 50 pounds of materials. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires fingering agility for keyboarding and using a telephone and other office equipment.
