What are the responsibilities and job description for the Program Coordinator - DD Waiver position at Colorado Family Caregivers?
Role: The Program Coordinator – DD Waiver manages a caseload of clients who receive residential services through the Developmental Disabilities (DD) waiver. The Program Coordinator supports individuals and caregivers by helping develop, implement, and monitor person-centered plans and supports that promote health, safety, independence, community participation, and quality of life. This is a remote from-home position that also requires regular visits to client homes and other service settings to monitor services, provide support, and ensure waiver requirements and individual needs are being met.
Responsibilities:
- Assist with the client onboarding process
- Educate, mentor, and mold care providers with the PRS person-center support plan
- Attend annual Service Plan meetings
- Oversee all aspects of the client care plan to ensure quality of services
- Ongoing monitoring of all services monthly to ensure services and support are being rendered as required by the members Service Plan.
- Complete quarterly in-person home visits as required
- Development and implement adaptive and/or behavior programming when applicable
- Other duties as assigned
Qualifications:
- Bachelor's Degree
- 2 years of work experience
- Experience with Colorado Long-Term Services and Supports Programs (DD waiver sepcifically)
- Experience servicing members on the DD waiver!
Compensation:
- $58,000 - $63,000 annual salary
