CO State Veterans Community Living Center at Rifle
The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouse and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. Description of Job
Colorado State Veterans Nursing Home at Rifle is a 89-bed, long-term care facility serving the nation’s veterans. It provides skilled nursing services 24/7 and maintains quality of life, safety, and effective care for a diverse and complex population of men and women with varied medical and psychosocial issues.
This House Supervisor position exists to provide 24-hour accountability for management and oversee the delivery of nursing services on a day-to-day basis. This position will assist in the coordination of care for residents who have acute/chronic health problems, physical and/or mental limitations and are unable to live independently or at a more independent level of care. This position participates in facility-wide oversight as designated by the Director of Nursing Services/designee.
Supervise and Resource Team
Supervise and direct subordinate staff in their delivery and directing of patient care. Examples: define job expectations, provide feedback in individual and group supervision, assess individual learning needs and provide staff training, resolve staff conflicts, act as a resource, refer staff to appropriate programs, prioritize and delegate job duties according to patient needs and staff qualifications, participate in job interviews, maintain compliance with standards of nursing practice, develop performance plans and complete appraisals (PMP), referrals for personnel actions. Maintain compliance with Colorado Department of Public Health and Environmental Services (CDPHE) and US Department of Veteran’s Affairs regulations.
General Management Collaboration
Command Crisis
Assumes leadership in crisis situations. Examples include: respond immediately to all psychiatric and medical emergencies, i.e., Code Red, accidents, injuries, fires and all other disasters; notify appropriate personnel of incidents, injuries; file appropriate reports in compliance with regulations, etc
Quality Assurance
Includes 24-hour accountability for delivery of nursing care for each neighborhood. Takes on-call rotation as defined in the facility protocol. Monitor quality improvement at shift and neighborhood level by: taking responsibility for the delivery of patient care according to nursing, nursing home and accreditation standards, compliance with charting by nursing staff (daily and weekly), keep staff informed of changes in nursing home and division policies. Quality improvement reports provided to the Performance Improvement Committee as assigned
Direct Nursing Care
Act as a Charge/Floor Nurse and assume all the duties of that role, including being on the on-call rotation, the treatment and care of a neighborhood unit and directing employees. Responsibility includes addressing attendance and performance/behavior issues and reporting the incident to their designated supervisor.
Continuing Education
Maintain updated skills, knowledge, abilities and competencies necessary to the position through continuing education, clinical supervision, literature review and participating in discipline and team development. Performs other related duties as assigned

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Minimum Qualifications:
Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state
participating in the Enhanced Nurse Licensure Compact (eNLC), in good standing.
AND
Three (3) years of professional experience as a registered nurse experience in an occupational field related to the work assigned to the position.
Substitutions
Conditions of Employment:
- Current and Active RN License renewal required.
- Must be willing to submit to a full background check
- This position requires travel to conferences and industry specific trainings/meetings.
- Must possess a drivers' license
- Must be willing to test for Tuberculosis (TB) test
- This position has shift work: day, evening and night shifts to include holiday and weekend as needed.
- This position requires on-call rotation as defined in the facility protocol.
Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
