Description Of Job

Under the guidance of the Case Manager III, job duties and responsibilities include but are not limited to:

  • As a work leader, oversee the work product of assigned Case Manager I staff, direct, train, oversee, mentor and coach. Provide backup and support as needed;
  • Create and maintain an interdependent approach with internal and external stakeholders to effectively guide offenders in accordance with CDOC needs, regulations, federal laws, state statues and other case management goals and objectives ensuring continuity of care from intake to release:
    • Admission, assessment and re-entry planning;
    • Implementation, progress assessment and modifications;
    • Parole planning and referral to community resources.
  • Utilize the tools of supportive inquiry and solution focused dialog to motivate offenders to be accountable for their facility programming and transitional needs;
  • Assist inmates with linking to programs and services within the facilities that address critical needs, such as, educational, medical or mental health issues, substance abuse, cognitive behavioral, offense specific treatment, etc. Each area requires an integrated case management team approach that utilizes multidisciplinary staffing models and offender buy-in to help develop and carry out offender case plans;
  • Oversee delivery of case management services as designed in policy and training, within their assigned facility that targets criminogenic needs through risk/needs assessments, case planning, and cognitive interventions;
  • Ensure that Case Managers I (CM I's) utilize motivational interviewing and cognitive based interventions, and that case planning is in alignment with Colorado Transitional Accountability Plan (CTAP);
  • Oversee management of inmate cases from intake to pre-release planning to ensure court ordered programs and conditions are met prior to release;
  • Maintain successful case management operations through tracking service delivery to offenders, and monitor:
    • Collection and evaluation of information to determine custody levels and programming needs;
    • Development of individual case planning through completion of assessments;
    • Documentation of inmate behaviors.
  • Provide individual guidance to (CM I's) to assist them with inmate-based service delivery;
  • Utilize a multidisciplinary and collaborative approach with internal and external stakeholders to effectively guide offenders in accordance with CDOC policy to ensure continuity of care from intake to release and ensure compliance with state statute and CDOC administrative regulations;
  • Educate, negotiate, and defend case management services and activities with internal partner programs, external agencies and the community;
  • Coordinate programming based on offender needs, determine earned time grants, develop parole plans, develop community corrections referrals, and act as the primary contact and source of information for facility administration, central administration, the Parole Board, outside agencies, attorneys, families, victims, and offenders.
  • Performs other job related duties as assigned or required.

Minimum Qualifications

MINIMUM QUALIFICATIONS: Four (4) years of *corrections experience which must include one (1) year providing case management services to incarcerated individuals. Case management services include case planning, determining custody level and programming needs, determining earned time grants, developing parole plans/community corrections referrals and linking the inmate to programs and services to address critical needs (experience less than full-time will be prorated).

Experience must be clearly described, in your own words, within the work experience section of your application to meet this requirement. Positions held within CDOC must be separated by classification, dates, and duties or the application may not be accepted.

*Corrections-related experience is defined as experience in which the employee provided a direct service to/with clients detained against their will such as custody/control, teaching, social work, nursing, food service, and general maintenance; or supervised offenders in the implementation of a program such as correctional industries. This experience is not typically earned in clerical support programs.*

Education and Experience: A combination of education in Criminal Justice, Social Work, Psychology, or a similarly related field and case manager experience as defined above equal to four (4) years.

PREFERRED QUALIFICATIONS (Candidates may receive higher ratings for possessing any preferred qualifications criteria.)

  • Relevant experience as defined above working with the State of Colorado;
  • Knowledge of Evidence Based Practices and Interventions for the successful re-entry of inmates;
  • Knowledge of inmate classification and job board in correctional facilities;
  • Extensive knowledge of law enforcement, corrections and parole operations;
  • Knowledge and experience with MPS to include Classification Committee, offender pay and multi-disciplinary staffing;
  • Experience with Parole Board planning and scheduling;
  • Knowledge and experience with auditing processes;
  • Broad knowledge of social perceptiveness, human behavior, attitudes, motivation and its applicability in the rehabilitation of inmates;
  • Training and experience with the CTAP Program (Colorado Transitional Accountability Plan).

Drivers must maintain a valid, non-restricted Colorado driver license, obey traffic laws, and be able to safely operate a motor vehicle. State drivers are expected to maintain a safe driving record consistent with Colorado Division of Human Resources Universal Policy Driving Standards . Eligibility to drive a State-owned vehicle, includes holding a valid Colorado driver’s license within 30 days of employment or residency change and meeting specific driving record criteria. Serious traffic violations, including DUIs or reckless driving, can disqualify drivers for up to seven years. A ll State Vehicles are monitored by telematics for safe operation and employees have no reasonable expectation of privacy while using an agency State Fleet vehicle.

Appeal Rights
