Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor vital signs and observe health changes to report to healthcare professionals promptly
- Administer medications accurately following prescribed schedules and protocols
- Support clients with memory care needs, including dementia and Alzheimer's care, using specialized techniques for behavior management
- Operate EMR (Electronic Medical Record) systems to document care activities and health updates efficiently
- Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary
- Use assistive devices such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance
- Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and sanitizing client spaces
- Drive clients to appointments or community activities in accordance with safety guidelines and licensing requirements
- Collaborate with team members to develop personalized care plans that promote independence and well-being
Experience
- 11 years Prior experience in home care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of long-term care practices, including resident’s rights and dementia or memory care techniques
- Hands-on experience with vital signs monitoring, medication administration, and emergency response procedures such as first aid
- Familiarity with EMR systems for documentation purposes
- Experience working with individuals with developmental disabilities or behavioral challenges is advantageous
- Valid driver’s license and safe driving record for transportation duties
- Strong communication skills to effectively interact with clients, families, and healthcare teams
- PCA certification required (or equivalent home care training)
- CNA
- CNA certification preferred if applicable
- High school diploma or GED required
