This position reports directly to the Program Administrator for SUD services but will work closely with the OSAR team, community resources and contracted substance use treatment providers. This position provides services to individuals age 13 and up who are seeking residential substance use treatment services.

This position’s responsibilities include assisting individuals that are eligible for residential substance use treatment services by addressing other personal needs, and by providing referrals and linkage to appropriate services. Tasks include facilitation of referrals for mental health, physical health, social, educational, housing, and other provider services and support that assist with meeting basic human needs, improve independent decision-making and self- confidence, support treatment completion, and engage in long-term recovery. The Case Manager may assist with transportation, scheduling appointments, and assisting the individual with picking up medications.

This position exemplifies service excellence by providing services with a trauma-informed and person-centered approach. Case Manager will have weekly contact with each individual (pre-entry to treatment, while on the waitlist for a treatment bed and during treatment into residential services) to ensure continuity of care. The Case Manager will document all services, referrals and follow ups in CMBHS system as outlined by HHSC. Documentation will be completed in a timely manner and accurately to reflect services being provided. Case Manager will track and document financial assistance provided to individuals and assist in reporting the required measures as outlined in the contract, if applicable.

This position practices good stewardship of resources by good utilization of workforce time, utilization of agency resources, leveraging of internal/external resources to meet the needs of the individual and creating value as a professional in knowledge, skills and abilities to perform these duties.

Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, Substance Abuse and Mental Health Administration (SAMHSA) grant, other applicable grants, and Lifepath Systems policies and procedures. This position will work closely with the Program Administrator for SUD services to ensure adherence to contract requirements is being met.

The Case Manager will build positive relationships with community-facing agencies and always maintain professional relationships with all community stakeholders and partners. This position demonstrates professional conduct with providers, peers, leadership and other support teams at all times.

The Case Manager will be expected to work independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals being served. This position through service delivery will assist in reducing barriers to treatment, enhancing motivation and encouraging treatment retention. This project will show improvement in measurable ways, which is central to LifePath Systems’ mission to serve as well as the purpose of this project. This position must also have the ability to assess and de-escalate behavioral health crisis situations in a calm and respectful manner.

Specific Targets:

SUD Comprehensive Case Manager will provide daily telephonic contact and at least one in person contact each week for those on the residential treatment waitlist; provide at least weekly contact with individual AND treatment team for those in residential treatment; create discharge plan in collaboration with residential treatment team at least 5 days before discharge that includes all necessary mental health, primary care, housing, and SDOH referrals; at minimum, Case Manager connects 95% of individuals to ongoing treatment and recovery support post residential treatment (ie outpatient, 12 step groups, or Recovery Support Services) within 1 week. Direct services are expected to be, at minimum, half of work time; documentation is to be completed within 48 hours of service

  • Must have a bachelor’s degree from an accredited college or university with major course work in a human services field as required to qualify as a Qualified Mental Health Professional (QMHP)
  • Preferred- 2 years’ experience working in the field of substance use disorder treatment and with individuals with a mental health diagnosis
  • English/Spanish bilingual preferred.
  • Must have reliable transportation and a valid driver’s license.
