The LPHA Access and Diversion Supervisor reports directly to the Diversion and Intensive Services Program Administrator within the Behavioral Health Division. This position will supervise the Access and Diversion QMHPs and the Continuity of Care Team. Supervisor provides regular individual and group coaching to all direct reporting workforce members as needed, to ensure fidelity requirements are being met, services provided are in accordance with all state, HHSC, TAC, and grant reporting requirements, and that workforce members are maintaining service provision and documentation standards. Supervisor ensures effective communication in all areas including provider operations and performance. Tasks include completion of intakes including biopsychosocial, financial assessment, needs assessment, recovery plan, TRR, and facilitation of initial diagnosis as well as completion of updated assessments and documentation needs for established individuals. Will perform diagnosis, crisis risk assessments, resource navigation and follow up services to individuals in services and needing to connect to services across the entire LifePath Systems continuum of care. Coordination and collaboration of interdisciplinary team meetings as needed, maintains oversight of the Inpatient Care Waitlist, and coordination between state and local psychiatric facilities. Provides person directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services with a focus on access to care and diversion from incarceration.
This position ensures connection between individuals from the community and psychiatric settings to LifePath Systems services. Uses initiative and a passionate commitment to provide access to care to individuals in services as well as diverting from emergency rooms, hospitals, and jails. Responsibilities include guaranteeing individuals receive timely access to appropriate services, working with the entire LifePath Systems team and other community organizations to identify needs and reduce barriers to improve social determinants of health. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. This position will provide clinical supervision to LPC-Associates as needed and perform other duties as assigned.

This position practices stewardship of resources by using time effectively with those served and seeks opportunities to expand access efforts. In addition, they ensure all required training is completed prior to the due date and center-issued equipment is kept in good repair. The Access and Diversion Supervisor will link individuals to additional resources needed to support their goals.
Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, UM guidelines, and Lifepath Systems policies and procedures. This position will work closely with Adult Fidelity managers and the Program Administrator to ensure that all gaps are reconciled.
The Access and Diversion Supervisor builds positive relationships with local police agencies, social service organizations, state and local hospitals, and mental health professionals to increase referrals from community stakeholders and partners. This position demonstrates professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities are required.

This position must have the ability to assess and de-escalate behavioral health crisis situations in a calm and respectful manner.
Specific Targets: Supervises team productivity, timeliness, schedules, and quality of services on a routine basis and ensures team members receive additional training as needed. Hiring new employees, completing performance reviews, monitoring and addressing performance issues, ensuring compliance, reviewing/approving time sheets and time-off requests, and arranging for adequate coverage of staff is expected. Works with PA regarding any administration needs resulting from team not fulfilling requirements. Conducts regular team meetings to promote cohesiveness and service oversight. Meetings have clear training agendas with sign-in sheets and minutes. Is responsible for meeting assigned performance metrics, completion of reporting requirements, monitoring services, assigning caseloads, and ensuring that services are provided in a welcoming, trauma-informed environment based on person-centered recovery goals. Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum of 20 hours per month. 90% of service notes are signed within 2 business days of service. 90% of full chart reviews receive a score of 80 or higher. Will maintain community-based calendar with stakeholders and partners information and report to supervisor weekly for utilization time and recommendation for future partnerships.
If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.
Qualifications:• Full, unrestricted Texas license as Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT); licensed associate or intern considered dependent on timeframe to full licensure.
• CCBHC knowledge and experience preferred.
• 2 years of clinical experience in the treatment of individuals with mental illness and/or substance abuse disorders required.
• 2 years leadership or supervisory experience highly preferred.
• Experience in Texas Recovery and Resiliency (TRR) preferred.
• Knowledge of Trauma-Informed Care Practices required.
• Must have reliable transportation and a valid driver’s license.
• Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention and symptom management services to individuals with serious mental illness or serious emotional disturbance.
• Ability to assess individual needs, to coordinate services, and to communicate effectively.
• Knowledge of the social services system and the ability to connect individuals with appropriate services.
• Ability to write clearly and professionally on all services and internal communications.
• Ability to think clearly and logically while dealing with potentially dangerous and/or emotional situations.
• Ability to respond to individuals with patience, empathy and understanding.
