Physical Therapist – Pay Per Visit
Location: Willing to relocate to Napa and Sonoma County
Work Setup: Pay Per Visit
Rate: $70 – $80 per visit (based on visit type – Start of Care, Discharge, Routine, etc.)
We are seeking a licensed and motivated Physical Therapist to join our home health team. In this role, you will provide one-on-one patient care in a home setting, helping individuals regain mobility, independence, and quality of life.
